<p>Breast reduction surgery is gaining popularity as a method of changing the body and the mind.  Large problematic breasts can be a real burden to bear, causing a variety of physical pains, social phobias and emotional issues.  Reduction mammoplasty is a drastic, yet highly effective potential cure for these troublesome concerns, yet there are many risks to consider before deciding if the procedure is right to fulfill your body image expectations.</p>
<p>Generally speaking, breast reduction surgery should only be considered if the size of the breasts is truly a real health concern and non-surgical reduction methods do not produce adequate results.  Diet and exercise can significantly reduce the size of borderline breasts, making surgical intervention unnecessary.  Women with a higher percentage of fatty tissue in their breasts will achieve better reduction results from noninvasive methods, while women with greater amounts of glandular tissue are unlikely to enjoy the same positive results.  Regardless of breast composition, breasts which remain too large after a real attempt at nonsurgical reduction might make the woman a good candidate for surgery.</p>
<p>Reduction mammoplasty is generally the most invasive of all breast plastic surgery procedures.  There are 3 primary forms of reduction surgery performed in the modern operating room and 2 of the 3 are rather challenging operations to endure.</p>
<p>Full breast reduction is also known as keyhole reduction, anchor incision reduction or wise pattern reduction.  This operation uses 3 separate, but joined incisions to remove interior breast fat and glandular tissue, as well as surgically place the nipple in a higher and more aesthetically pleasing location on the breast mound.  The surgery utilizes a periareolar incision, a vertical incision from the bottom of the areola to the inframammary fold and an inframammary incision across the natural breast crease.  Full reduction is used for the largest and most ptotic of all breasts.</p>
<p>Vertical breast reduction, also known as lollipop reduction, is a less invasive version, utilizing only 2 joined incisions.  The technique uses a periareolar incision surrounding the nipple/areola complex and a vertical incision from the bottom of the areola to the natural breast fold.  This approach saves the patient from enduring the pain, healing time and possible degenerative effects of an inframammary incision.  Vertical technique is used on breasts which can be adequately reduced using a less invasive approach.</p>
<p>Finally, breast liposuction is a minimally invasive option for women who do not require a full or vertical reduction surgery.  Liposuction is effective at removing large amounts of breast fat which has not dissipated through diet and exercise alone, but is not a good choice for women with large glandular breasts.  Liposuction is a perfect choice for women who want to be smaller, but do not need to undergo full open breast surgery.</p>
<p>Finding the best reduction approach for your breasts is worth the time and effort spent consulting with more than one cosmetic surgeon.  Skill levels and experiences differ amongst surgeons and surgical recommendations are likely to vary as well.  Find a doctor who can offer you the best results using the least invasive procedure and make sure to investigate the surgeon’s credentials and past operative history before committal.   Above all else, make sure you connect with the doctor emotionally to be sure that they will satisfy your goals 100%.</p>

Breast implants both saline and silicone have been approved by the FDA, and are considered safe to use. Maintaining safety includes annual checkups with your surgeon, self exams, annual MRIs, as well as possible replacement years later.</p>

After the patient decides for breast enlargement, she must be fully aware of the risks and side effects of having a foreign object in the body.</p>

Scar tissue that hardens and distorts, called capsular contracture, can occur. If this occurs, then the patient may have to undergo more surgery. The individual may also have increased pain in their breast or decreased nipple sensation. An additional risk that occurs often is that of a leak.</p>

Silicone Leak:</p>

If the individual&#8217;s silicone gel-filled implants rupture, silicone gel leaks outside of the silicone shell. Silicone leaks can be intracapsular or contained within the scar tissue surrounding the implant. It can also be extracapsular which means the silicone leaks outside the scar tissue.</p>
<p>&#13;<br />
At times, silicone ruptures are difficult to detect with a mammogram alone. It is possible that the person could have a silent rupture meaning you will not have any outward signs of the leak. While some women experience subtle changes in the size, shape or contour, magnetic resonance imaging (MRI) is usually necessary to identify any problems and to determine if it is intracapsular or extracapsular.</p>
<p>&#13;<br />
Because intracapsular can become extracapsular if not addressed in a timely manner, a cosmetic surgeon will usually recommend that ruptured implants be removed and replaced. Because silicone leaks can be difficult to detect by just a physical examination, women with enlargement are encouraged to have MRI exams every two years, starting a few years after surgery. These exams might not be covered by health insurance.</p>
<p>&#13;<br />
Most women with silicone gel-filled breast implants will have at least one broken within ten years of the plastic surgery procedure.</p>

Saline Ruptures:</p>

If a saline rupture, saline leaks out of the shell. A problem will be very obvious because your breast implant will become deflated, changing in size and shape as the saline solution leaks outside the shell and into the rest of your body. Most of the time, leaked saline solution is easily and safely absorbed by the body if detected earlier.</p>
<p>&#13;<br />
If an individual later chooses removal, they may experience dimpling, puckering, wrinkling or other changes of the breast. Depending on many factors including incision technique used, breast implants may affect your ability to produce milk for breast-feeding, especially if there is a rupture.</p>

In Case of Rupture:</p>

If your implant leaks or breaks, the plastic surgeon will need to remove it with surgery. This procedure can typically be completed through the same small incisions cut during the initial surgery. Most of the time, the individual can have a new device inserted during the same surgery.</p>

The procedure usually is straightforward, but at times leaking silicone gel can be difficult to remove. The surgeon may require more extensive cosmetic surgery. Additionally, health insurance may not cover implant removal or replacement and therefore you may be out of pocket quite a bit of money.</p>

This complication is frequently caused by infection, hematoma, or seroma. It will result in firmness and distorted breast shape; you will most likely experience some pain as well. 			<content:encoded><![CDATA[<p>The non-cosmetic clinical indications for the use of breast implants are for breast reconstruction, sex reassignment surgery, and for abnormalities, usually congenital, that affect the shape and size of the breast. Of the breast plastic surgery procedures performed in 2006 in the United States, 329,000 were for breast augmentations. Insertion of breast implants is also known as breast augmentation, breast enlargement, breast enhancement, mammoplasty enlargement or augmentation mammoplasty.</p>
<p>&#13;<br />
In the 1970s plastic surgeons wanted softer and more lifelike implants so breast implants were redesigned with thinner gel and thinner shells; these implants had a greater tendency to rupture and leak, or &#8220;bleed&#8221; silicone through the implant shell into the body cavity and complications such as capsular contracture were common. It was predominantly silicone implants that were designed in the 1970s that were involved in the class-action lawsuits against Dow-Corning and many other manufacturers in the early 1990s. One manufacturer did produce a model of pre-filled saline implants but has been reported to have had high failure rates following surgery.</p>
<p>&#13;<br />
In the mid 1980s, advances in manufacturing principles brought about elastomer-coated shells to decrease gel bleed, and were filled with thicker, more cohesive gel; these implants are sold under restricted conditions in the U.S. and Canada, but are widely used in other countries. Saline implant shells are made of silicone elastomer and the implants are filled with saline solution after the implant is placed in the body. Dr. Thomas Cronin and Dr. Frank Gerow, who were two Houston, Texas, plastic surgeons, developed the first silicone breast prosthesis with the Dow Corning Corporation back in 1961.</p>
<p>&#13;<br />
Saline-filled breast implants were first manufactured in France in 1964 and were introduced by Arion with the goal of being surgically placed using smaller incisions than they were then using. The first woman was implanted with silicone implants in 1962.</p>
<p>&#13;<br />
Since the early 1990s, a number of independent reviewers have examined studies concerning links between silicone gel breast implants and systemic diseases; the consensus of these reviews is that there is no clear evidence of a causal link between the implantation of silicone breast implants in breast plastic surgery and systemic disease; it&#8217;s always wondered who is funding these studies and that should clearly be stated. There is a general international consensus in the medical field that silicone implants in breast plastic surgery have not been shown to cause systemic illness, excluding the possibility that a small group of patients may become ill through (as yet) unknown mechanisms may prove difficult; this has been questioned by many women experiencing various health issues.</p>
<p>&#13;<br />
Extracapsular silicone has the potential to migrate to other parts of the body, but most clinical complications have appeared to be limited to the breast and axillae as inflammatory nodules (granulomas) and enlarged lymph glands in the armpit area called axillary lymphadenopathy it&#8217;s reported.</p>
<p>&#13;<br />
One study reported 30% of ruptures in asymptomatic patients are accurately detected by experienced plastic surgeons, compared to 86% detected by MRI. An intracapsular rupture can progress to outside of the capsule, called an extracapsular rupture, and it is generally agreed that both conditions indicate the need for removal of the implant. Manufacturers and doctors are required to inform women that implants are not  permanent devices and that most recipients will probably need additional surgery to replace or remove their implants.</p>
<p>&#13;<br />
When breast implants are removed that have been implanted for a long period of time, a mastopexy is often performed to tighten up the loose skin: this is additional surgery usually done at the same time and at an additional expense. Studies of saline-filled breast implants approved by the FDA in May 2000 showed rupture and deflation rates of 3-5% at three years and 7-10% at five years for breast augmentation patients.</p>
<p>&#13;<br />
The age and design of the implant are  important factors in rupture, but estimating rupture rates of contemporary devices has been difficult for a variety of reasons, mainly because implant designs have changed somewhat over time. Capsules of tightly-woven collagen fibers form as an immune response around a foreign body like breast implants, which tends to wall it off; capsular contracture happens when the capsule tightens and squeezes the implant.</p>
<p>&#13;<br />
Some doctors in specialties other than plastic surgery, like dermatologists, perform breast plastic surgery like breast augmentation; choose a plastic surgeon instead. If you decide to proceed, make sure you choose the best breast plastic surgeon based on your research; ask health professionals you know for referrals. They also may have a problem with sagging of what little breast tissue they have. This is more common in women who have had children and in women who had had significant fluctuations in weight.</p>
<p>&#13;<br />
Evaluation of these patients includes looking at the texture of their skin, to see if there are stretch marks, noting significant underlying damage to the skin&#8217;s ability to tighten . In addition, it&#8217;s important to look at the shape of the breast, the areolar size, the asymmetry of the breasts, and the age of the patient.</p>
<p>&#13;<br />
1. The truth about implants. Contrary to what you may think, the majority of plastic surgeons would choose silicone over saline for patients. This prevailing opinion is agreed upon by 9 out of 10 plastic surgeons.</p>
<p>&#13;<br />
Currently, the implant manufacturers are using gel that is even thicker. They call it &#8220;cohesive gel&#8221; because it is designed not to leak out in case of a disruption in the outer shell.</p>
<p>&#13;<br />
As the size of the implant increases, it becomes more difficult to disguise it. Women are selecting larger implants than ever before and they need to know that their body can only camouflage a certain size implant before it becomes obviously &#8220;un-real&#8221;. My consultation with every patient includes a discussion that specifically compares their body and breast type with the implant size that they choose.</p>
<p>&#13;<br />
2. &#8220;What happens to my breasts with age?&#8221; Breast tissues age just like other tissues of the body. The skin stretches out, the fibers that hold the internal tissues together weaken, the milk-producing gland tissue enlarges and shrinks with each menstrual cycle and pregnancy, and the fatty tissue varies with weight changes.</p>
<p>&#13;<br />
Insertion of a small or moderate breast implant behind these tissues will be accommodated well and will improve the shape of the breast in most cases; however, larger implants are also heavier, and they will produce a greater degree of stretch to the tissue, thereby causing it to weaken over time.</p>
<p>&#13;<br />
Brassiere sizing is not constant. Although the brassiere manufacturers set up a standard sizing system, they do not adhere to it. A 34C bra bought from Victoria&#8217;s Secret is much smaller than a 34C bought from Target. We are, however, accustomed to describing breast size in terms of bra size.</p>
<p>&#13;<br />
It is better to discuss your desires with a surgeon based on the look that you want rather than the bra size. Bring in photographs of topless models, ask to place sample implants inside a bra that gives you the look that you are hoping for, or visit websites that show examples of patients that have undergone breast augmentation.</p>
<p>&#13;<br />
3. &#8220;Is the surgeon peer reviewed?&#8221; Peer reviewed means that the surgeon has privileges to do breast augmentation at a hospital or surgical center where other surgeons also operate at. To obtain those privileges, the surgeon must submit an application that is reviewed by other qualified surgeons. These other surgeons have been selected to give privileges only to new surgeons that perform skilled surgery.</p>
<p>&#13;<br />
A board certified plastic surgeon can only be certified in America by the American Board of Plastic Surgery. Other boards exist that certify dermatologists, facial plastic surgeons, cosmetic surgeons, gynecologists etc. They are not truly plastic surgeons although they may perform plastic surgical procedures and may call themselves plastic surgeons.</p>
<p>&#13;<br />
&#8220;Should a non-plastic surgeon do my surgery?&#8221; If you decide to work with a non plastic surgeon, you should check up on the surgeon&#8217;s qualifications.</p>
<p>&#13;<br />
How many years has the surgeon been in practice? It is always reassuring to know that the surgeon has been practicing for some time, and that he performs a considerable amount of surgery. How many cases does the surgeon do per year? If the surgeon is using his own operating room, you must ask if it is certified by one of the accepted certifying agencies. These organizations maintain standards that ensure safety, quality and experience.</p>
<p>&#13;<br />
4. &#8220;Who puts me to sleep?&#8221; There are several methods used for administering anesthesia. If an anesthetist is supervised by an anesthesiologist (M.D.) on the premises, you have nothing to worry about. If the anesthetist is supervised by the surgeon or by an anesthesiologist that is not physically present, you should think twice about surgery performed in that location.</p>
<p>&#13;<br />
You want your surgeon to focus on your surgery and not to be distracted by the responsibility of the anesthesia. Some rural hospitals allow anesthetists to provide anesthesia without supervision. Personally, I would not allow an unsupervised anesthetist to give me anesthesia, unless it was a dire emergency.</p>
<p>&#13;<br />
Anesthesia is a drug induced mental state that allows surgeons to work without you experiencing pain. These drugs can and often have other effects. Some drugs may cause postoperative nausea and vomiting. Usually this will resolve in a day or two, although occasionally patients say that they remain mildly affected for up to two weeks after surgery.</p>
<p>&#13;<br />
Newer drugs and techniques are developed every year, usually because they do the job better and with fewer side effects. It is the sole job of the anesthesiologist to keep up with these drugs and techniques, so that your anesthesia goes as smoothly as possible.</p>
<p>&#13;<br />
5. &#8220;Why do prices vary?&#8221; The cost of the surgery is a combination of three charges. The first of the charges is the surgeon&#8217;s fee. The surgeon&#8217;s fee will include the operation and all of the office appointments. Do not underestimate the office appointments.</p>
<p>&#13;<br />
Most surgeons will allow you to return annually at no charge to check up on your implants and to examine your breast tissue. Take advantage of this benefit. If you go out of town for your surgery, you are giving up that benefit. The surgeon&#8217;s fee will also include the price of the implants. Some surgeons include medications while others ask you to fill a prescription. This can amount to $100-200.</p>
<p>&#13;<br />
The second charge is the operating room fee. Surgeons that use a surgical center or a hospital will tell you that fee, and you will usually pay it directly to the facility. Surgeons that use their office for the surgery will not usually describe this fee specifically; it will be part of the &#8220;inclusive fee&#8221;.</p>
<p>&#13;<br />
Food for thought: Surgeons that run their own operating room have been tempted to cut corners to save expenses. Again, be sure that the operating room has been certified.</p>
<p>&#13;<br />
The third charge is the anesthesia fee. Usually the same billing methods apply as for the operating room.</p>
<p>&#13;<br />
Why is the cheapest quote definitely not the best? There are some things that add cost to the surgery, which are well worth the extra expense. Yes, you can have a breast augmentation surgery without these things, but you will be sacrificing something. The sacrifice may be a safety feature or it may be a less optimal appearance of your breasts.</p>
<p>&#13;<br />
Allow me to give some examples. There are many types of implants. Silicone implants cost more than saline implants. Some implants have a separate saline and silicone compartment. Implants are shaped differently. These are for special situations. They are more expensive to produce and therefore they sell for a higher price.</p>
<p>&#13;<br />
Your surgeon should tell you about these special implants if he feels that they will be a better choice for your particular situation. Your surgeon should also tell you if your breasts will require additional work, such as a breast lift, to obtain the optimal result. This additional work will add more time to the surgical procedure and therefore the cost of the surgery will increase.</p>
<p>&#13;<br />
An anesthetist is not as costly as an anesthesiologist. Some surgeons cut costs in their own operating room by using anesthetists. Always consider the &#8220;what ifs&#8221;, because surgery is not without risk. Be comfortable that your surgeon is well trained and strives to have as few complications as possible. Complications do occur and will often lead to additional expenses.</p>
<p>&#13;<br />
If you go out of town for your surgery, you will have to travel again to treat a complication should it develop. Or you will have to go to another surgeon closer to home to treat the complication. It is the best policy to choose your surgeon wisely. Try to avoid shopping for the lowest price surgeon.</p>
<p>&#13;<br />
6. &#8220;Do I want to buy the extended warranty?&#8221; Did you know that all of the implant manufacturers have a warranty for their implants and that they sell an extended warranty for an additional fee? As I have said above, no medical devices including breast implants last forever. Breast implant manufacturers offer a very generous warranty.</p>
<p>&#13;<br />
They realize that their product is chosen as a lifestyle luxury. You will not see this type of warranty from any other type of medical device manufacturer. The breast implant manufacturers will replace the implants, if anything goes wrong with them, for your entire lifetime.</p>
<p>&#13;<br />
In addition, for several years after implantation they will also reimburse some of your expenses for the other costs of the second surgery. The extended warranty that can be purchased increases the amount of reimbursement and lengthens the period of time that the reimbursement is available to you. So how do you decide if it is worthwhile to buy the extended warranty?</p>
<p>&#13;<br />
Most consumer advocates will agree that extended warranties are generally not worth the expense, and merely serve to make greater profit for the seller. However, there are always certain situations that can make it worthwhile. I will tell you my personal opinion regarding the extended warranty for breast implants.</p>
<p>&#13;<br />
Most plastic surgeons want to have happy patients that will return to them in the future. If one of our patients has an implant problem, such as a leaky implant, also called a deflation, we will usually perform the second surgery, even if it is many years later, for a minimal fee. However, if another surgeon performed the first operation, the surgical fee for the second surgery can be higher than the original surgery.</p>
<p>&#13;<br />
Therefore, if you think that you may not be able to return to your original surgeon in the future, e.g. you have plans to move elsewhere, it may be prudent to purchase the extended warranty. Conversely, if you traveled a distance to an outside surgeon, you may want to consider purchasing the extended warranty.</p>
<p>&#13;<br />
Remember the warranty only applies if there is a problem with the implant. If in the future, you wish to change implants for a larger size, you will have to pay for the new implants.</p>

The conventional wisdom is that women have breast augmentation mostly for vanity, but many women suffer from serious malformities in their breasts that can cause them severe anxiety and self-consciousness in all situations requiring undress, from public showers to changing rooms to intimate relations.  For these women, even looking at themselves in the mirror becomes an unhappy event, and can lead to serious depression and low self-esteem, but breast augmentation can be used to correct a number of serious breast deformities to let women live a normal life without such a high degree of self-consciousness.</p>
<p>&#13;</p>
<p>Tubular Breasts<br />&#13;</p>
<p>Tubular breasts are characterized by narrow bases, often high on the chest wall.  The breasts, though narrow, sometimes have a significant amount of tissue, which extends forward, but, unable to support itself, shows a high degree of ptosis (sagging).  In addition, these often have unusually large areola, which are distended outward by a protrusion of the breast tissue under the nipple.  </p>

<p>Inexpert augmentation of this kind of breast can lead to a phenomenon known as double-bubble, or double-breast where the breast tissue sits visibly atop the implant, but a skilled cosmetic surgeon can correct this malformity by reshaping the breast using a donut mastopexy (breast lift), a reduction of the areola, and a reshaping of the breast tissue to give a more natural, pyramidic shape to the breast.</p>

<p>Downward Pointing Breasts<br />&#13;</p>
<p>In many cases, downward pointing breasts may just be the result of ptosis of the breast, and can be corrected using a breast lift, with or without augmentation.  However, in some women, downward pointing breasts are the result of a prominence of the upper chest wall.  This women can achieve a good shape of the breast following augmentation if high profile implants are used that can sit in the midchest, under the prominent upper chest protrusion, balancing the breast and making it more natural in shape.</p>

<p>Divergent Breasts<br />&#13;</p>
<p>Women with divergent breasts have a hard time achieving good cleavage in most outfits.  The separation of the breasts can make them appear even smaller than they are, especially impacting their projection in front of the chest bone.  For these women, breast augmentation can bring the breasts closer together and reposition the nipples, giving a more normal, appealing shape.</p>

For example,some people call it breast augmentation surgery while doctors and surgeons call it mammoplasty surgery, and others may call it breast lift or breast enhancement surgery. Whatever name it is called, breast enlargement surgery is a surgical procedure to enhance the size and shape of a woman&#8217;s breast for a number of reasons such as :-</p>
<p>&#13;a)To enlarge women&#8217;s breast to make them bigger.</p>
<p>&#13;b)To correct a reduction in breast volume after giving birth.</p>
<p>&#13;c)To balance breast of uneven shapes or sizes.</p>
<p>&#13;Plastic cosmetic surgeons are able to increase a woman&#8217;s breast by inserting implants into each breast to make the breast look bigger and firmer.</p>
<p>&#13;Breast enlargement surgery can enhance the appearance of your breast and can bring you more self confidence which is a boost to your self esteem. Before you decide to have a breast surgery, do carefully consider your expectations, do some basic research and discuss your plans with your plastic surgeon.</p>

<p>&#13;A breast implant is a silicone shell filled with either silicone gel or saline. About concerns that there is inadequate information on the safety of silicone gel-filled breast implants, the Food &amp; Drug Administration (FDA) has determined that new gel-filled implants should be available only to women participating in approved studies and research. However, saline-filled implants for breast enlargement are mostly unrestricted by legislative law.</p>
<p>&#13;As with any surgery and operation, there are health risks involved although they are rare. The most common problem is the capsular contracture. This occurs if the scar or capsule around the implant begins to tighten. The squeezing of the soft implant makes the breast feel hard and look artifical. Capsular contracture can be treated in many ways although sometimes it requires the removal or replacement of the implants.</p>
<p>&#13;Excessive bleeding following the operation may cause some swelling and pain. If excessive bleeding continues, another operation may be needed to control the bleeding and remove the blood accumulated from the bleeding.</p>
<p>&#13;Some women may develop an infection around an implant although it is uncommon to develop such infections. The infections may occur at any time, but is most often noticed within a week after the breast enlargement surgery is performed. In some rare cases, the implants may need to be removed for a couple of months until the infections clear up and then new implants can be re-inserted.</p>
<p>&#13;There are some fears that breast implants may affect fertility, pregnancy, or the ability to nurse babies. However, these fears are unfounded as there are no evidence to substantiate them.</p>
<p>&#13;Sometimes, breast implants may leak or even rupture. A rupture can happen as a result of an injury or from the normal compression and movement of your breast and implant, causing it to leak. If a saline-filled implant ruptures, the implant will deflate but the saline water will be harmlessly absorbed by the body.</p>
<p>&#13;A rupture of a gel-filled implant may cause some complications and that is why the FDA put restrictions on this type of implants. If the shell breaks but the scar capsule around the implant does not, you may not detect any change in your condition.</p>
<p>&#13;If the scar also breaks or tears, the silicone gel may move into surrounding tissues. The gel may collect in the breast and cause a new scar to form around it, or it may migrate to another area of the body. There may be a change in the shape or firmness of the breast.</p>
<p>&#13;Both types of broken implants may require the patient to go for another surgery to replace of the damaged breast implant. In some cases, it may not be possible to remove all of the silicone gel in the breast tissue should gel-filled implant ruptures resulting in more complications for the patient concerned.</p>
